Telmisartan + Amlodipine

Tablet

Telmisartan + Amlodipine is a fixed-dose combination used in the management of hypertension. Telmisartan, an angiotensin II receptor blocker (ARB), lowers blood pressure by blocking vasoconstriction. Amlodipine, a calcium channel blocker, relaxes vascular smooth muscle. Together, they provide dual antihypertensive action and cardiovascular protection.

Strength
  • Telmisartan 40 mg
  • Amlodipine 5 mg
Packing Size
  • 10 Tablets

Mechanism

Telmisartan: Blocks angiotensin II receptors, preventing vasoconstriction and aldosterone release, lowering blood pressure.
Amlodipine: Inhibits calcium influx into vascular smooth muscle, causing vasodilation and reducing peripheral resistance. The combination ensures effective blood pressure control and cardiovascular protection.

Pharmacokinetics

  • Absorption: Telmisartan ~50% bioavailability; Amlodipine ~60–65%.
  • Distribution: Both highly protein bound.
  • Metabolism: Telmisartan minimal hepatic metabolism; Amlodipine via CYP3A4.
  • Elimination: Telmisartan mainly fecal; Amlodipine renal and fecal.
  • Half-life: Telmisartan ~24 hrs; Amlodipine ~30–50 hrs.

Side Effects

  • Dizziness
  • Peripheral edema
  • Headache
  • Fatigue
  • Rare: Hypotension

Contraindication

  • Hypersensitivity to Telmisartan or Amlodipine
  • Pregnancy (especially 2nd and 3rd trimester)
  • Severe hepatic impairment
  • Cardiogenic shock (Amlodipine contraindicated)

Interaction

  • Other antihypertensives (additive effect)
  • CYP3A4 inhibitors (increase Amlodipine levels)
  • NSAIDs (may reduce antihypertensive effect)

Warning and Precautions

  • Use under medical supervision only
  • Monitor blood pressure regularly
  • Caution in renal impairment
  • Consult physician during pregnancy and lactation

Dosage
